What is Patient-Centered Care?

Updated: Jun 17

At Compass Community Health, our mission is simple: to provide access to quality healthcare to all members of our community (see our About page to learn more about our mission). Our focus is YOU. We know that each one of our clients is unique, and that is why we offer patient-centered care. But what does “patient-centered” mean for YOUR care?

According to the New England Journal of Medicine, patient-centered care can be defined as,

“In patient-centered care, an individual’s specific health needs and desired health outcomes are the driving force behind all health care decisions and quality measurements. Patients are partners with their health care providers, and providers treat patients not only from a clinical perspective, but also from an emotional, mental, spiritual, social, and financial perspective.”

Notice the word hidden in the middle of that definition- ‘partners’. As a partner with your healthcare provider, your specific needs and preferences are top priority. Not only will you not have to struggle to feel heard or seen, we ENCOURAGE you to advocate for yourself; your voice helps shape your care. People are multi-faceted, and so there is no cookie-cutter appointment. We care about building relationships, and we take every facet of an individual into consideration to provide comprehensive, quality care, just for you.

Among the many services that Compass offers, there is:

  • Primary Care

  • Mental Health

  • Pediatric Occupational Therapy

  • Pediatric Speech Therapy

  • Women’s Health

  • Pharmacy

  • Payment Assistance

  • and more services!

Are there circumstances under which you feel that a doctor’s appointment just wouldn’t really work for you? We’ve thought of that too! We offer:

  • Extended hours to fit your schedule

  • Same day and next day appointments

  • A transportation shuttle for patients who may have trouble getting to their doctor’s appointment

  • Coordinated care where outside appointments and information is organized and communicated to patients
